Meet Our MIH Community Partners

ADMIT - Rockford

The ADMIT - Rockford group is an amazing group of professionals who meet each month at various host facilities in the area. ADMIT - Admission and Discharge planners, Marketing and other healthcare professionals, who meet and develop Ideas Together, is a wonderful network of organizations whose purpose is "to increase education and information while promoting a quality continuum of care". 

An educational program is offered as part of their monthly meeting and includes time for attendees to meet and greet and promote the program they represent. They also host a Fast Track, a day-long opportunity to visit multiple facilities in the Rockford area, and a Fall Conference.

The MIH program has been active in ADMIT for over two years. Trent Brass was the presenter at their September meeting in 2017.

The ADMIT meetings are held the first Wednesday of each month from 8 am - 9 am.To check the schedule of upcoming meetings please go to their Facebook page.There is no charge to attend the monthly meetings and no membership fee.

Learn More About Second First Church
Second Congregational/First Presbyterian Church

The strongest asset we have for the Mobile Integrated Healthcare Program are the resources that exist in the community. The connections we make through the resources we know are amazing and, most certainly, show up at exactly the right moment. 

That was exactly how we met Reverend Rebecca White Newgren, Senior Past at Second Congregational/First Presbyterian Church. We were looking for overnight resources for some of our patients - she was looking for resources to be a part of an Overnight Warming Cafe! 

While there is still work to be done to get the Overnight Cafe open during the winter months Reverend Newgren and her congregation came together to open their facility during the extremely cold stretch and it was a great success!

We are looking forward to working with Second First Church on this and other community projects.

Right from the beginning Heartland Hospice has provided support and education to our MIH Managers. We will always be grateful to Tammy Christensen and her team for being so welcoming. The Heartland team has been patient with us as we learned how important these difficult conversations with patients can be and all about the services that can be provided by a hospice program.

In addition to hospice care, Heartland also provides a palliative care program called Advanced Disease Management. 

This program coordinates care for patients with chronic medical conditions and/or terminal medical conditions. The Nurse Practitioners who are part of this program assist patients in navigating not only their healthcare needs, but also needs related to diet, spirituality, and therapy based on the patients' diagnosis.

National Alliance on Mental Illness of Northern Illinois
NAMI, the National Alliance on Mental Illness, is the nation’s largest grassroots mental health organization dedicated to building better lives for the millions of Americans affected by mental illness. NAMI Northern Illinois serves Boone, Stephenson, Winnebago and the surrounding northern Illinois counties. Their members are families, friends, professionals, advocates and people living with mental illness. They provide education, offer support and resources, and advocate for those who need our assistance.
